Application Requirements
1.
Driver's License/or Florida ID Card
2.
Social Security Card
3.
Proof of Citizenship
Need 1 of the following:
- Passport,
- Resident Card,
- Birth Certificate,
- Work Permit
4.
Employment Physical Exam
- Dated within the last 6 months and states you are free of any communicable disease.
After the 2-weeks certification course, work as a caregiver at a facility, hospital, with homebound patients, the disable, or anywhere caregiver's services are needed.
